I am selling this as this was dads.  This is the EZ Lite cruizer from 2016.  We did upgrade this 2 years ago to the stronger wheels, since yes they did not last  There is There is the headrest, foot rest, back pack, battery bag, premium battery and an extra battery for back up.  A little warn, yet in great working condition, folds easy, and is about 50 60 pounds depending whats in back pack. Batteries can be charged off-board.  Adjustable arm and foot rests. Back rests reclines in 5 positions. Purchased from manufacture, I have all the paper work trails and all info my dad kept when researching the chair. Removable seat cover for easy cleaning.
